What is the next step?

When you call Interventional Spine Medicine, our coordinator will ask you about your medical history and help answer your insurance questions. Prior to your visit, you’ll receive a registration packet with forms you should fill out before coming in for your appointment.

What should I bring to my first appointment?

Bring your completed New Patient Registration Packet, your insurance card, insurance referral (if required by your insurance company), a picture ID and any recent images on the CD ROM of the MRIs, x-rays or CT scans if you have them.  Please also bring your current Medication List.

What can I expect on my first visit to Interventional Spine Medicine?

During your appointment we will do a complete medical history, you will be given a physical examination, your ISM pain management expert will talk to you about next steps in your diagnosis and treatment.

What insurance plans do you accept?

We accept the following insurance plans: Aetna, Anthem – Blue Choice/Blue Cross of New Hampshire, Cigna of New Hampshire, Harvard Pilgrim, Medicare, Tufts, Martins Point and Tricare.
